Modern Reformation collects and stores your personal information when you create an account, sign up to receive our e-newsletter, or sign our guest book. When creating an account, we ask you for your first and last name, your mailing address, e-mail address, and phone number. If you are placing an order or making a contribution we will also ask you for payment information and, if applicable, shipping method.
